NOTE: A course cannot be approved for online delivery until it has been fully approved for inclusion in the master catalog. If the proposed course is not currently in the catalog please provide a copy of the Faculty Senate approved course recommendation (for Undergraduate courses) or the Graduate Council approved course recommendation (for Graduate courses) when you submit this form. Approvals or non-approvals must be returned to the FDCOMI chair.
