I, the undersigned, have read through the modules related to TA training and fully understand them. I have attended the TA training and agree to follow the rules as outlined in both the on-line and physical training. I also agree that if I fail to follow the rules or continually violate the rules, the Chemistry Department may take any action it sees fit up to and including my dismissal.
