Forms, Instructions, and Approvals

In filling out these forms, ensure that you (or your mentors) are complete and explicit to avoid delays. If you have any questions, contact Dr. Mallory.

It is critical that both you and your mentors fully understand the guidelines of any project you wish to perform. When working on proposals, ensure that clear, written expectations are communicated on the forms. Additionally, Shadowing Capstone students must print out a copy of the guidelines, have your mentor sign it, then return it to Dr. Mallory with the other materials.

Research Capstone

Research Capstone Proposal Form - Deadline: Submitted to your mentor and approved before the start of classes for the semester in which the work will be performed.

Research Capstone Project - Deadline: Submitted to your mentor before the beginning of exam week for the semester in which the work will be performed. (Note, there is no Research Capstone Evaluation Form).

Shadowing Capstone Instructions and Deadlines - A full list of requirements for you and your Clinical Mentor.

Shadowing Capstone Proposal Form - Deadline: Submitted to Dr. Mallory and approved before the start of classes for the semester in which the work will be performed.

Clinical Mentor's Instructions and Acceptance Form - Deadline: Must be signed by mentor and submitted to Dr. Mallory prior to initiating the shadowing activities.

Shadowing Capstone Mentor's Evaluation Form - Deadline: Submitted to Dr. Mallory with paper report before the beginning of exam week for the semester in which the work will be performed. The shadowing student is responsible for ensuring that their mentor receives this form in sufficient time to allow them to complete it before the deadline.

Research Independent Study

Independent Study Proposal Form - Deadline: Approved by Dr. Mallory before the start of classes for the semester in which the work will be performed.

Application forms for Research Support

Research Undergraduate Tuition Waiver Application - Deadlines 15 April for fall waiver and 15 November for spring waiver, submit to Biological Sciences Office (S350).

Research Undergraduate Grant Application- Deadlines 15 April for fall support and 15 November for spring support, submit to Biological Sciences Office (S350).
