The following lists include important forms, documents, and other resources for those involved in the Assessment and Program Review processes. If there are any documents that feel are missing, please let us know by email at assessment@marshall.edu and we will work on getting it included on this list.
Programs should use the following forms when preparing the Annual Assessment Reports. Actual copies of the Annual assessment reports may be found by clicking on the 'Assessment Reports and Program Reviews' link to the left. Assessment Committee members should use the Committee forms when reviewing the reports. Forms to assist with syllabus development and review are located at the bottom of the page.
